8. How can I make a claim?
 (1) Application for a claim for accidental death
Upon application for a claim for accidental death, the applicant shall submit the following materials:
①Application for payment of claim;
② Original of the insurance policy or certificate;
③ Identification certificate of the insured;
④ Identification certificate of the insurance beneficiary;
⑤ Written confirmation of the death of the insured issued by a medical institution or post-mortem report acceptable to the competent public security department or the insurer;
⑥ Declared death certificate issued by a people’s court of the People’s Republic of China (excluding Hong Kong, Macau and Taiwan regions) in case the insured is declared dead;
⑦ Household register record cancellation certificate issued by the Public SecurityDepartment or cremation certification issued by a relevant department;
⑧ Accident report issued by a relevant department authorized by Public Security or traffic laws and regulations;
⑨ Other evidence and data for identifying the nature, cause, and extent of damage of the accident covered by the insurance.
 (2) In case the applicant is the heir of the insured or the beneficiary, a relevant legal entitlement certificate shall also be provided.
 (3) If the insurance claim applicant appoints another person to apply for the insurance claim, a corresponding letter of authorization, and identification certificates of both parties shall be provided.
 (4) Application in case of an overseas incident
In case of an overseas incident, in addition to the provision of the corresponding application documents specified in (1)–(3) above, documents issued by any overseas institution or person shall be notarized regarding their validity and truthfulness by a legal notary organization at the place of the overseas occurrence, or acknowledged by the local Chinese embassy or consulate.
9. Notice for claims
 (1) Reporting time: The insurance applicant, the insured or the beneficiary shall notify our company within 10 days ofthe incident covered by the insurance liability.
 (2) If the insurance applicant, the insured or the beneficiary intentionally omits or fails by reason of major negligence to give notice- thereby resulting in difficulties identifying the nature, cause or damage extent of the accident covered by the insurance liability- the insurer shall not be held liable to pay any claim regarding any unidentifiable part thereof, unless the insurer has timely knowledge of the accident, or can identify the nature, cause and damage extent of the accident though the insurer was not notified in a timely manner.
